Jack W. Welburn, Jr., 64 of Worcester died Tuesday, Oct. 4, in UMASS Memorial Hospital.
He leaves four sons; Christopher Welburn, Sean Welburn, Eric Welburn, and Mark Welburn, his former wife, Gail (Terry) Welburn; three grandchildren, Hannah, Joshua, and Lillian; a brother, Paul Welburn; two sisters, Suzanne McEnaney and Avis Hendrickx; several nieces and nephews and a dear friend of six years, Sue Arsenault. He was born in Woburn, son of Jack W. and Mary (Hamilton) Welburn. He lived over 30 years in West Boylston.
Jack was the founder and owner of Wel-Serve, Inc. He was a member of the Harold N. Keith, American Legion Post and enjoyed the outdoors, golfing, playing cards, and camping.
The funeral will be held Friday, Oct. 7, at 11 a.m. in FAY BROTHERS FUNERAL HOME, 1 W. Boylston St., W. Boylston. Burial will be in St. John's Cemetery. Calling hours are Thursday, Oct. 6, from 4 to 8 p.m. at the funeral home.
